  3. Complete the questionnaire and email it back to us at CustomSmoothies@LauraHarrisSmith.com

Laura will research your needs, create your personalized smoothie and email it to you herself! (Current wait time is 6 weeks.) If you experience difficulty downloading your questionnaire, email us at the above address and we will send it directly to you by email. Then email it back to the same address.

  3. We will send the recipient an email stating they have been gifted a Custom Smoothie™ from you! You will be cc:d on that email (using the email from your order/payment info).

  4. That email will contain their health questionnaire which they will complete and return to us, after which they will receive a second email from us with their Custom Smoothie™ recipe card (again, cc:d to you).
